If directory to watch is a symlink, currently it is not followed. It'd be nice if there is a warning about this, or a mention in the doc, or an option is added to resolve symlink.

Patches are definitely welcome. Did you test this behavior in multiple watcher implementations? I wonder if inotify and kqueue behave differently than the pure Perl code.

I suspect the right thing to do is just to always follow symlinks.
