Description:

A form control on the closed page has more than one label associated with it.

Steps to Reproduce:

  1. Navigate to the closed page.
  2. Inspect the form control and its associated labels using developer tools.

Expected Behavior:

A form control should have only one label.

Actual Behavior:

A form control has multiple labels.
