[Bug]: Occasional repeated user messages + duplicate agent work

Describe the bug and reproduction steps

Every once recently I get user messages duplicated like this. I only typed the message once but it gets posted twice.

The worse thing about this is it seems to actually maybe get the agent working twice in parallel, I'm getting messages that seem to be from agents doing independent work. It's a very weird bug...

Unfortunately this doesn't seem 100% reproducible so I'm not sure how to isolate the problem, but I thought I'd post it here so we can start working on a fix. I think it might happen more often after a runtime error, or possibly after I stopped the agent.
